Job Description:
Description The Social Services Clinician MSW provides services
to patients and their families/caregivers to meet their social and
emotional needs as they relate to their medical diagnoses and
concerns. They collaborate with the multidisciplinary team to
ensure that patients receive treatment, recovery, and safe and
coordinated transition from one care environment to another. They
work to protect and advocate for the rights of patients and ensures
that they receive highest practicable physical, mental and
psychosocial well-being of each patient. The Social Service
Clinician MSW works under the direction of Care Management
Leadership and is employed by Providence Health & Services. The
position will practice in accordance with competencies and
guidelines outlined in Standards for Social Work Practice in Health
Care Settings and the Code of Ethics as defined by the National

- Master's Degree: Degree as a Social Worker from an accredited
School of Social Work. Preferred Qualifications:
- Certification in Oncology Social Work (OSW) in Oncology
Setting
- American Case Management Certification (ACM)
- 2 years Medical Social Work experience.
- Experience with computerized documentation and medical records
